Simple Scheduling To Meet Your Needs
To request an appointment with one of our Pawnee Valley Community Hospital providers, we offer the following three convenient options:
- Request an appointment online. Simply complete the form below at your convenience. Requests received after normal business hours will be processed promptly.
- Make appointments through your Patient Portal at any hour. It’s easy to access your patient portal to request an appointment. Simply login or create an account anytime, 24 hours a day, 7 days a week.
- Feel free to call us anytime during business hours. Our family medicine clinic is open on weekdays for appointments from 8 am to 12 pm and then from 1 pm to 5 pm, except on Tuesdays when we have extended hours and are open until 7:30 pm. For more information or to schedule an appointment, call us at 620-804-6007 during weekday office hours. For all other appointments, please call that clinic or provider directly to schedule your appointment.
If you are having a medical emergency, call 911 or go to the nearest emergency room. If you need medical advice by phone, please contact your regular care provider.
If you need to cancel your appointment, please notify us at least 24 hours in advance by calling the clinic you have an appointment with.
