Case Management is available to our patients for assistance with a variety of issues, including discharge plans to ensure you receive the care and community support you need after leaving the hospital.

Pawnee Valley Community Hospital is staffed with a licensed social worker who is available to assist patients and families in a variety of situations. The social worker is available to:
Assist patients and families in identifying ways to cope with the psychosocial aspects of illness and chronic disease
Help with developing and implementing individual discharge plans to ensure patients receive the care they need after they leave the hospital
Assist patients and families in obtaining needed community resources such as: transportation, durable medical equipment and oxygen, home delivered meals, homemaker services, mental health and/or substance abuse counseling, hospice care, medical alert systems, prescription assistance and nursing home and assisted living placements
Support patients’ rights to participate in making decisions about their healthcare by providing education and assistance in completing advance medical directive forms such as a Living Wills and Durable Power of Attorney for Healthcare Decisions
The Pawnee Valley Community Hospital Social Worker is available to patients and families upon request or on referral from any member of the healthcare team.
