Our Diabetes Solutions program gives people with diabetes the tools and knowledge needed to better manage their disease. You need the support of your full diabetes team – your primary provider, diabetes educator, dietitian, exercise physiologist, behavioral health advisor, and care partner – to achieve optimal control of diabetes. Education is key to empowering you to better manage your diabetes.
Educational Classes
Diabetes Solutions is a comprehensive educational program accredited by the American Diabetes Association. The class content includes information about:
Diabetes
Healthy eating
Being active
Monitoring blood sugars
Diabetes medications
Problem solving
Reducing risks
Healthy coping
This is a comprehensive program that consists of two, five-hour sessions. Each session runs from 8:45 am to 2:30 pm and includes lunch. You are not required to attend consecutive classes in the same month.
A physician referral is required and participants must submit registration two weeks prior to the class (printable class schedule or view the Diabetes Solutions class). Getting into the class is easy: ask your doctor to fill out this referral form, sign it, and fax it back to us at 785-623-5108.

Program Goals
Our Diabetes Solutions program aims to:
Help participants become active in their own care and diabetes management
Provide avenues for patients to be successful in achieving self-management goals
Ensure our program focuses on the national standards of care according to the ADA and the American Association of Diabetes Educators (AADE)
Provide interactive learning and up-to-date information on diabetes self-management
Provide education to caregivers so they feel comfortable being a part of the patient’s support team (caregivers are encouraged to join patients at no additional cost)
